To work across the domain of Urgent Care, including the Emergency Department, Acute Medical Unit, Acute Frailty Unit, and Same Day Emergency Care (SDEC).To work with the Multi-disciplinary Team to promote an efficient, effective service for patients and carers.Work as an autonomous practitioner, both professionally and legally accountable for all aspects of the professional and clinical work.Focus therapeutic intervention for symptoms and difficulties associated with presenting diagnosis; being guided by recommendations from up-to-date NICE Guidelines.To have responsibility for the therapeutic management of patients within the speciality care pathway.To be responsible for a designated caseload of patients, where you will assess patients, implement and evaluate treatment plans, and coordinate therapeutic interventions.You will assist in the organisation, development, promotion, and delivery of the Service.To liaise with health, education, and social services, working in partnership with primary and secondary care, social prescribers, and other external agencies to ensure the delivery of high-quality care.Make recommendations to the team leader for changes to practice, lead the implementation of specific changes as required, and contribute to service guidelines.Main duties of the jobPerform advanced holistic triage and assessment of patients with multi-system symptom presentation associated with complex physical, emotional, and psychological health.Develop and deliver an individualized treatment programme.Work collaboratively with primary and secondary healthcare services, third sector services, social prescribing, education, and social services to ensure seamless, high-quality service delivery.Manage a caseload and work independently, with indirect supervision from the clinical lead via formal training, 1:1 sessions, peer review, and case studies. Support and advice from the MDT is available.Provide support to the MDT and cover for absent colleagues as required.Undertake all aspects of clinical duties as an autonomous practitioner.Act as an advocate for patients and their families when necessary.Carry out risk assessments in line with Trust/National Protocols.Develop patient-centered intervention and management plans anticipating care needs.Ensure all interventions are evidence-based and adhere to professional standards and NICE guidance.Keep up to date with professional standards, recent literature, and guidelines.About us We hold a unique position in the Stockport community as the provider of healthcare and are one of its largest employers.We aim to improve workforce diversity to reflect our community.
