The service will enable people to receive an intensive community rehabilitation service closer to home in their own community. We are transforming with the NHS Long Term Plan (2019) for Mental Health at the heart of our innovations.The community rehabilitation service will consist of a multi-disciplinary, multi-agency workforce, working collaboratively between Rotherham Doncaster and South Humber NHS Foundation Trust (RDaSH) and North Lincolnshire Council to share skills and specialisms to support individuals and improve their quality of life.Main duties of the jobMAIN DUTIESTo work within an occupational therapy mental health pathway aimed at supporting people to achieve their recovery goals/ functional stabilityTo carry out assessments and interventions, utilising the OT process, from initial assessment to discharge planning and evaluation.
