The ISF is a multidisciplinary group of experienced clinicians committed to delivering high-quality person centred care. The team support CYP with a diagnosis of Autism Spectrum condition and or Learning Disabilities who are at risk of admission to a mental health hospital and or placement breakdown.We work collaboratively with wider agencies, child/young people and their families to manage complexity, mitigate risk, and achieve meaningful outcomes through the use of tailored therapeutic intervention whilst applying the underpinning principles of positive behavioural support.We operate a small shared caseload affording team members the opportunity to work intensively and dynamically to meet the needs of our client group.We Seeking Enthusiastic Practitioners WithA recognised qualification in nursing, social work, or allied health professions.Experience working with CYP facing mental health challenges or who are experiencing behaviours that may challenge.Knowledge and experience of using Positive Behavioural Support ModelStrong communication, interpersonal, and organisational skills.A commitment to ethical practice and continuous professional growth.Be part of a supportive, innovative team offering ongoing training and development. Youll have the chance to grow professionally while making a real impact on young peoples lives.What Youll Bring To The RoleEnthusiasm and Dedication:A genuine commitment to supporting children and young peoples mental health and behavioural challenges.Clinical Expertise:Experience in multi agency working to meet the needs of CYP with a complex behaviour and/or mental health profile.Coordination and Case management:The ability to lead several complex cases through a evidence based clinical pathway and to have experience in supporting junior team member to undertake functional assessments, observations and to deliver therapeutic interventions.Risk Management:Experience supporting CYP who present with clinical risk due to mental and emotional health needs.Resilience Under Pressure:Confidence in managing a fast-paced, dynamic environment with flexibility and adaptability.Positive Behavioural Support Plan development :Experience in the development and implementation of positive behavioural support plansPracticality and Mobility:Ability to travel to various venues including CYP homes or education provisions as required throughout the working day.Why Join Us?We offer the ability to demonstrate creativity and commitment to the work at hand whilst affording shared ownership of risk and intervention planning when supporting CYP with a complex behavioural and mental profile through the composition of a shared caseload.
