This innovative post will be cross-sector providing support and care to palliative patients at RWT, Compton Care hospice and patients in the community. The post holder will participate in multidisciplinary team working on wards and in clinics, delivering high quality, cost-effective and patient-focused pharmaceutical care. The individual will act as a clinical role model within the Pharmacy department and supervise junior and pharmacy support staff at ward level. The post holder will be responsible for the delivery and further development of pharmacy services to the Palliative Care Directorate and Compton Care. Compton Care has an 18 bedded inpatient unit, virtual ward, a 24/7 rapid response, planned care community team and day therapies that aims to provide first class palliative care to all who need it, wherever they are. Education is a priority and there is a proactive approach to learning and development. The palliative care teams are research active and supports both individual and collaborative projects. There are ample opportunities to develop your leadership abilities and interest in the specialty. The successful candidate will be encouraged to develop their own skills through mentorship, training and education. Main duties of the jobTo provide clinical and directorate support to the palliative care department at RWT. To provide a comprehensive specialist pharmaceutical service to patients and under the care of Compton Care (inpatients and community). To provide clinical input to the palliative care virtual wards To ensure a seamless service for patients moving between settings. To work as an independent prescriber managing patients with specialist palliative care needs. To provide specialist clinical pharmacy advice to palliative care patients and liaise with the consultants and nursing teams to support safe and appropriate prescribing in this patient group. Support the strategic development of the clinical pharmacy service within the palliative care specialty To continuously monitor the drug expenditure against budgets for the hospice and trust. To manage any palliative care related switch schemes that may arise, in conjunction with the wider pharmacy and palliative care teams ensuring as far as possible a safe, efficient and comprehensive service. To ensure appropriate governance within the palliative care directorate and Compton Care. To support the development of relevant local policies, procedures, guidelines and clinical care pathways for the locality.
