Work both independently and collaboratively with a team of nurses and healthcare assistants to coordinate patients throughout their pre-assessment pathway, with a high level of input into the management and care of patients prior to admission for specified procedures. Act as a resource and provide education for patients and healthcare personnel, and direct the pre-assessment nursing team. Carry out relevant forms of care without direct supervision, working effectively within the multi-disciplinary team. Support all members of staff within the Outpatient department, including nursing students and visiting healthcare professionals. Participate actively in the teaching and mentoring of students and junior staff. Suggest improvements in care provision, recognize and challenge poor practice, and raise concerns to relevant agencies where appropriate. Ensure that the nursing philosophy of care for pre-operative assessment is reflected in practice. Provide a high standard of nursing care to patients attending the Outpatient department, working within agreed policies and procedures. Act as a liaison with the Multi-disciplinary team and contribute to meetings such as the Anesthetic Meeting / Complex MDT meeting, escalating complex patients and helping plan their care. Collaborate and consult with the multi-professional healthcare team and patients, communicating sensitive and complex information. Use highly developed communication skills to guide, advise, and support patients, ward staff, physiotherapists, and theatre staff regarding management of admissions and their impact on patients and carers. Identify and overcome barriers to patient understanding when communicating complex, sensitive, and sometimes distressing information. Provide verbal and written information to enable patients to give informed consent for treatment, using specialist knowledge. Establish partnerships with patients and carers, communicating in highly distressing or emotional circumstances. Interpret and communicate complex information with healthcare personnel. Act as a patient advocate, ensuring informed consent, privacy, and dignity are maintained (NMC). Carry out health promotion using information leaflets and networking with other healthcare staff within the Trust where necessary. Critically reflect on own performance through supervision and action learning.
