Colliery Court Medical Group are looking for an enthusiasticPractice Pharmacist with General Practice experience to join our friendly team. We are an innovative and forward-looking practice of 8,250patients, working in a well-equipped purpose built premises. We embrace skill-mix with an extensive team, including GPs,pharmacists, advanced nurse practitioners, practice nurses, first contactphysiotherapists, mental health practitioners, podiatrists, social prescribinglink workers and health care assistants. We are a training practice (GP registrars and medicalstudents) and provide weekly teaching and real time supervision to ourhealthcare professionals. We support the professional development of our team. Main duties of the jobUp-to-date clinical knowledge, a thorough understanding of primary care prescribing, medication reviews, experience of designing and implementing strategies for improving prescribing. The ability to thrive when working in a devolved, flexible, autonomous environment. Have excellent written and verbal communication skills, and experience of conducting face to face and remote clinical caseloads in primary care. Maintain a good knowledge of QOF, enhanced services and other parts of the GP Contract, CQC clinical regulatory standards, IIF and other areas. Work alongside ICB and PCN pharmacists and liaising to ensure no duplication in workstreams. Processing prescription requests including acute requests Processing incoming documents with medication amendments and monitoring requirements. Responding to Tasks and emails in both the clinical System (Optum EMIS) and Outlook. Processing lost prescription replacement requests, reviewing the appropriateness of re-issuing another prescription. Run monthly Medicines Safety searches. A key responsibility of the post holder will be conductingclinical medication reviews to proactively manage patients with complex issues,ensuring their treatment regimens are both safe and effective. The post holder will serve as a primary point of contact forgeneral practice staff, providing expert advice on prescription and medicationqueries. They will play a crucial role in supporting the repeatprescription system, handling medication queries and medication reviews. The post holder will take a role in medicines optimisationand quality improvement, overseeing aspects of the quality and outcomesframework as well as enhanced services, (alongside the PCN pharmacy team) Communication with community pharmacies is a vital part ofthis role. Person SpecificationQualificationsMasters Degree in Pharmacy Registered member of the General Pharmaceutical Council with no restrictions to practice. Independent prescriber. Experience of working in a GP practice or PCN.
