The PCN CareCo-ordinator plays an important role within a PCN to proactively identify andwork with people to provide coordination and navigation of care and supportacross health and care services. The postholder will deliver NHSHealth Checks in line with national guidance and provide administrativefollow-up to ensure continuity of care. They will also support general hubcoordination and front, patient navigation, and population health initiatives. The role includes participation in arota covering extended access hours to include two weekday evenings (currently Wednesday andThursday until 8 pm) andSaturdays (9am 5pm) Main duties of the jobNHS Health Check Delivery oDeliver NHS Health Checks for eligible patients,carrying our near point of care testing, including lifestyleassessments, and cardiovascular risk scoring. oProvide brief interventions and health promotionadvice tailored to individual risk profiles. oAccurately document clinical data in patientrecords using appropriate systems. Administrative Follow-Up oManage post-check communications, includingresults, follow-up bookings, and referrals. oEnsure accurate coding and data entry in linewith NHS Health Check programme requirements. oSupport reporting and audit processes tomaintain service quality Care Coordination & Hub Support oAct as first point of contact for patientsaccessing the Health & Wellbeing Hub (face-to-face, phone, email). oBook patients into appropriate services andsignpost to community resources. oPromote hub services and events, including viasocial media platforms.
