We are seeking an enthusiastic registered nurse with some experience in diabetes to join our dynamic team. The role involves clinical management of people with diabetes in a variety of community settings, including community clinics, care homes, and home settings. The community diabetes specialist nurse team has a strong commitment to delivering excellent diabetes care and knowledge.This role is essential in providing expert care, education, and support to patients with diabetes across the community setting.You will work autonomously and collaboratively to assess, plan, implement, and evaluate person centred care.
