Join to apply for theCardiac Rehabilitation Nurserole atEast Lancashire Hospitals NHS Trust 1 day ago Be among the first 25 applicants Join to apply for theCardiac Rehabilitation Nurserole atEast Lancashire Hospitals NHS Trust An exciting opportunity has arisen for a band 6 Cardiac Rehabilitation Specialist Nurse to join the team at East Lancashire NHS Trust.You will be joining an accredited service delivering cardiac rehabilitation to patients who have had a cardiac procedure, event or a certain cardiac conditions such as heart failure.Your role will be to provide early phone consultations with patients, offering reassurance and support, delivering face 2 face community clinics, observing and participating in the exercise classes and education sessions. Working flexibly to cover clinics at short notice to meet the needs of the service . Supporting patients in accordance with the British Association of Cardiovascular Prevention and Rehabilitation (BACPR), and the "NHS Commissioning standards for cardiovascular rehabilitation", adopting a personalised approach to patient care that maximizes independence, choice and control, enabling people to reach and maintain their optimum level of health and well being.You will be joining a dedicated team of nurses who will support you to achieve the standards expected to deliver a quality service.
