Hourly rate :£38 per hour Location: Slough Hours & Days to be Worked: Full Time Essential Requirements : HCPC registered , 3 years Social Services Experience Job Description: Supporting with Trusted Assessments where required and using clinical expertise to screen and process hospital discharges for the Discharge to Assess pathway.
