They will work collaboratively with the team to meet the needs of patients, following policy and procedures to deliver high quality responsive care. The primary responsibility of the HCA Navigator will be to provide an initial assessment of patients attending the service including, confirmation of identity and presenting complaint, determination of appropriate clinical stream and priority. If navigated to Mastercall PCC, vital signs will be recorded including, temperature, pulse, blood pressure, respirations, oxygen saturation, blood glucose and urinalysis as required. Patient clinical priority will be established utilising a set of tools validated for use within the service. The post holder will liaise with the senior clinician on duty to facilitate urgent clinical assessment as required. The role is based within Trafford Primary Care Centre at Trafford General Hospital and the HCA will work as a member of the clinical team, supporting registered healthcare professionals in the delivery of an unscheduled primary care service delivered by Mastercall Healthcare. Employee benefits include: Living Wage Employer NHS Pension Scheme NHS Blue light card/Discount scheme Flexible working Career development Competitive rates of pay Generous holiday entitlement, long service enhancements and the option to purchase additional l eave Wellbeing schemes Job descriptionJob responsibilitiesHCA duties will include, but are not limited to: Support to the clinicians on duty A range of clinical tasks such as recording vital signs, blood sugar recording and urinalysis as needed Chaperoning duties when needed After appropriate training, to conduct face to face navigation assessment (for patients presenting to the service with appointments and walk in patients) Preparation and maintenance of clinical areas including appropriate stock control Record the tasks for which you have received appropriate training using electronic systems Use early an appropriate warning score (eg NEWS2) to ensure a deteriorating patient is escalated to the appropriate senior person in a timely manner in accordance with policy and procedure Give accurate and appropriate information to patients, relatives and colleagues within the sphere of own competence Administer basic life support and use of AED if necessary Support health professionals in the assessment and surveillance of patients health and well-being Support health professionals with awareness of health and well-being and how it can be promoted Assist with the collection and collation of data on needs related to health and well-being Proactively communicate with the team to ensure the smooth running of the shift. Be responsible for prioritisation of consultations as clinically indicated within the shift. Work with colleagues to maximise effective and efficient delivery of care to patients. Record appropriate consultation details on electronic patient records. Liaise with other local services such as the urgent care centre (UCC) should patients require transfer to these services. Follow Mastercalls policies and procedures. Communicate effectively with patients and carers, recognising their needs for alternative methods of communication Communicate effectively with other team members and with the wider Mastercall teams. Continuous Professional Development . The post holder will be expected to undertake ongoing personal, clinical and professional development in line with the responsibilities of the post. Responsible for developing and sustaining own knowledge, management, and skills (as appropriate) and professional awareness and maintain a professional profile.
