Job summary Hounslow PCNs are looking for a Lead first contact paramedic to join our growing multi-disciplinary PCN teams working across the five PCNs in Hounslow. The PCNs are Brentworth, Chiswick, Feltham & Bedfont, Great West Road and Hounslow Health. Hounslow is a diverse outer London Borough of approximately 324,000 registered patients supported by 47 GP Practices. The Hounslow Consortium brings together the borough's five Primary Care Networks (PCNs) to work collaboratively for the benefit of patients and their practices. Hounslow is traditionally considered an underserved area and several initiatives have been under way to increase the appeal of working in Hounslow through training and development of staff, technology and our partnership with Hounslow and Richmond Community Healthcare NHS Trust. We are seeking individuals who are passionate about improving care for patients living in areas of high socioeconomic deprivation, tackling health inequalities which have been exacerbated by COVID-19 pandemic and increasing access to high quality multi-disciplinary general practice. We will provide support and coaching for you to help you deal with challenges that this may present. Main duties of the job This is an operational lead role, working alongside clinical directors to establish new and innovative ways of working to maximise the opportunities that PCN working offers. The role will give you a solid grounding in Primary Care, working across a PCN footprint and closely alongside other ARRS roles. Our Lead Paramedics will work as a team to shape and deliver the future care for our patients who receive their care at home, working in collaboration with our Integrated Community Response Service who are leaders in admission avoidance with the aim to maximise our impact on the health and well-being of our local population. You will have the opportunity to be involved in service improvement, implementing initiatives and directives locally with the support and supervision of your clinical leads within the PCN. As a Lead Paramedic you will be expected to contribute to the leadership of the PCNs but also as a group of five work together to lead services in all the PCNs. You will be expected to provide leadership and mentoring to your team as well as develop effective relationships with the GP mentors for your staff. You must have access to your own transport, as this role will require travelling both to visit patients in their homes but also to a number of PCN practices and colleagues across Hounslow.
