You’ll be part of a dynamic team working across the borough to empower carers and connect them to the services they need.As an Assistant Care Coordinator, you’ll play a vital role in supporting carers over the age of 18, providing tailored advice, emotional support, and practical assistance. You’ll carry outstatutory carers assessmentsandsocial care assessments under the Care Act 2014 , ensuring carers’ needs are identified and met in a timely and person-centred way.You’ll also support ourfront-of-house duty function , helping totriage and screen referrals , respond to initial enquiries, and ensure carers are directed to the most appropriate support.Key Responsibilitiescomplete statutory carers assessments and Care Act 2014 social care assessments provide one-to-one support and guidance to unpaid carers assist in developing and implementing personalised carer support plans support front-of-house duty to triage and screen incoming referrals coordinate and participate in outreach and engagement activities maintain accurate records and contribute to service monitoring collaborate with health, social care, and voluntary sector partnersOur ideal candidate will haveexperience in health, social care, or community support knowledge of the Care Act 2014 and assessment processes excellent communication and interpersonal skills a compassionate, person-centred approach ability to work independently and within a team strong organisational and IT skillsBoth posts are full time - 37 hours per weekIf you would like to find out more about the role contact Nicola McGregor - Team Manager on 01384 818723 or email Council is committed to safeguarding and promoting the welfare of vulnerable groups and expect all staff and volunteers to share this commitment.Adults Enhanced DBS Check - We are committed to safeguarding and promoting the welfare of vulnerable adults; therefore, the post is subject to medical clearance and an enhanced DBS check.So that we can demonstrate Dudley’s commitment to diversity and inclusion and to improve bias in the recruitment process that could create barriers for candidates.
