The role can be tailored to the individual’s needs and interests, as acute medicine is a broad specialty. This role is ideal for a GP seeking to diversify and expand their clinical experience.The candidate would work in the SPoA hub and possibly in SDEC (Same Day Emergency Care), Frailty-SDEC, or Hospital outreach. Details of each include:SPoA: a local hub providing MDT (Community, Acute, and Hospital at home) referrals for patients in crisis, working closely with primary care, community teams, and hospitals to help patients stay at home.Acute Hospital at Home (AHAH): a virtual ward providing hospital-level treatments at home using wearable tech, diagnostics, and community assessments, currently serving 0-0 patients annually.SDEC: providing rapid assessments, diagnostics, and management in a single visit, promoting a patient-centered approach.Hospital Outreach: visiting patients at home, focusing on frailty or those with dependents, providing treatments like oxygen, IV medicines, and monitoring.About the RoleThe successful candidate will provide medical input to the frailty virtual ward alongside geriatricians, community clinicians, and nurses.
