Adult Social Care Hospital Senior Practitioner Role Overview The Adult Social Care Hospital Senior Practitioner plays a vital role in enabling safe and prompt discharges from hospital, ensuring individuals receive suitable social care support within the community. This role involves intricate casework, leadership, and teamwork with multi-disciplinary teams. Key Responsibilities ● Complex Casework Management: Supervise and support social workers to Coordinate a caseload of both complex and straightforward cases, where individuals have diverse needs, such as physical disabilities, learning disabilities, mental health conditions, and dementia. Ensure thorough assessments are submitted for Quality Assurance. ● Safeguarding Adults: Act as a lead professional in safeguarding adults at risk of abuse or neglect, conducting enquiries and coordinating interventions to ensure their safety and well-being. ● Discharge Planning and Coordination: Work closely with hospital staff (doctors, nurses, therapists) to ensure smooth and efficient discharge processes. This involves identifying social care needs early, advocating for patients, and coordinating necessary community support. ● Supervision and Mentorship: Provide professional supervision, guidance, and support to less experienced social workers and review officers. This includes case discussions, performance feedback, and professional development. ● Multi-Agency Collaboration: Develop and maintain strong working relationships with various internal and external stakeholders, including health services, housing departments, voluntary sector organisations, and independent care providers. ● Policy and Practice Development: Contribute to the development and implementation of adult social care policies and procedures, ensuring compliance with relevant legislation and best practice guidelines. ● Crisis Intervention: Respond to and manage crisis situations, providing immediate support and developing short-term solutions to prevent escalating issues. ● Record Keeping and Reporting: Maintain accurate and up-to-date records, ensuring all documentation meets statutory and organisational requirements. Prepare reports as required for management and external bodies. Essential Skills and Qualifications ● Qualified Social Worker: Must hold a recognised Social Work qualification (e.g., Degree in Social Work, DipSW, etc). ● Professional Registration: Registered with Social Work England. ● Extensive Post-Qualifying Experience: Significant experience working in adult social care, preferably within a hospital or reablement setting, demonstrating expertise in complex case management and safeguarding. ● Knowledge of Legislation: In-depth knowledge of relevant social care legislation, including the Care Act 2014, Mental Capacity Act 2005, and Safeguarding Adults guidance. ● Assessment and Care Planning Skills: Proven ability to conduct comprehensive assessments, formulate person-centred care plans, and implement effective interventions. ● Leadership and Supervisory Skills: Demonstrated ability to lead, mentor, and support other professionals. ● Communication and Interpersonal Skills: Excellent verbal and written communication skills, with the ability to build rapport and communicate effectively with diverse individuals and professionals. ● Problem-Solving and Decision-Making: Strong analytical and problem-solving abilities, with the capacity to make sound decisions under pressure. ● Resilience and Adaptability: Ability to work in a fast-paced and demanding environment, managing competing priorities and adapting to changing circumstances.
