ANP or Paramedic working in General PracticeWe are looking for a Advanced Nurse/Clinical Practitioner, on a permanent basis, to do 24 hours per week on a Monday, Tuesday and Wednesday (8 hours a day -days are non-negotiable ).You will be based at Clifton Road Surgery, situated within the centre of Rugby. We are a large and forward looking training practice responsible for approximately 15,000 patients. Main duties of the jobWe inviteapplications from suitably qualified Practitioners with skills to clinicallyassess, diagnose, refer and manage patients with a range of conditions. Theability to work independently and to demonstrate critical thinking in theclinical decision-making process is required and to take lead responsibilityfor the care of patients as well as working as part of a team. All our appointmentsare clinically triaged, ensuring our patients see the right clinician at theright time. All our Advanced Nurse/ClinicalPractitioner appointments are 10 minutes in length as the patient presentingwill have one single acute issue. Admin and results time is protected, together with access to a regular twoweekly mentor time with a dedicated GP. The Practice usesEmis Web and training will be available for the successful candidate ifrequired. About usYouwill be based at Clifton Road Surgery, situated within the centre ofRugby. Ourpractice team consists of 4 GP partners, 1 Managing Partner, 5 salaried GPs, primary care practitioners,practice nurses, HCAs, plus a full team of admin support. Job responsibilitiesSummary The primary focus of the role is to work as an autonomous practitioner providing high quality and timely patient centred care. The post holder will be responsible for the assessment, diagnosis, treatment, referral and discharge of a diverse range of patients with minor injuries/minor illnesses, without reference to a doctor and within their level of competence and agreed protocols. Primary Duties and Areas of Responsibility Clinical Act as an ANP/PCP, demonstrating advanced clinical competence and a knowledge base beyond those associated with conventional ANP/PCP roles. Make professionally autonomous decisions for which he/she is accountable. Provide safe, evidence-based, cost-effective, individualised patient care. Offer an holistic service to patients and their families, developing (where appropriate) an on-going plan of care and support, with an emphasis on prevention and self-care. Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills. Undertake consultations with patients as an autonomous practitioner and using own clinical judgement to diagnose, treat, refer and/or discharge patients. Duty Practitioner Rota cover which involves on the day support to patients with urgent acute medical needs. Make direct referrals to secondary care and social services within locally agreed pathways, guidance and protocols. Provide clinical advice to Reception staff where required. Ensure that patients receive high quality clinical care, delivered in a timely manner. Supply and administer medicines as indicated to address patient need. Ensure complete and accurate documentation of each and every patient contact. Work as an autonomous practitioner and as part of multi-disciplinary and multiagency teams in order to ensure patients needs are met. Refer patients directly to other services/agencies as appropriate in line with Practice policy. Work with multi-disciplinary team within the Practice to promote integrated and seamless pathways of care. Contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery. Help develop and set up new patient services and participate in initiatives to improve existing patient services. Communication and Working Relationships The post-holder will be expected to communicate with staff at all levels throughout the organisation as well as patients and carers. Of particular importance is the development and maintenance of effective communication and working relationships with, and between, multi-disciplinary agencies. Professional Role Adhere to the Code of Professional Conduct and at all times work within the scope of professional practice. Ensure that professional practice adheres to organisational policies, procedures and guidelines. Maintain a professional manner and act as a positive role model for staff and other members of the practice Team. Maintain confidentiality with regard to information pertaining to patients and staff. Promote evidence-based practice through the use of the latest research-based guidelines and the development of practice-based research. Monitor the effectiveness of own clinical practice through the quality assurance strategies such as the use of audit and peer review. Maintain their professional registration. Participate in continuing professional development opportunities to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role is maintained. Record accurate consultation data in patients records in accordance with the latest NMC guidance and other pertinent standards. Keep up to date with pertinent health-related policy and work with the practice team and, where appropriate, with the wider locality team, to consider the impact and strategies for implementation.
