Your responsibilities will include: Carrying outphysical health assessmentsand monitoring, including phlebotomy duties and has to be phlebotomist trained and able to work under pressure Supportingsafety planningand participating inmulti-agency meetingsto ensure coordinated, high-quality care. Main duties of the jobYour Main Duties Will Include: ? Co-designing and delivering personalised care plans in collaboration with experienced staff. ? Providing practical and emotional support to individuals throughout their recovery journey. ? Conducting physical health assessments, including phlebotomy and monitoring vital signs. ? Supporting safety planning and contributing to multi-agency discussions to ensure coordinated care. ? Encouraging and facilitating engagement with local community resources and services. ? Conduct home visits where necessary using own transport or public transport with the use of a Lone working device and safe portable bag provided by the Trust ? Promoting wellbeing and empowering individuals to live meaningful, independent lives. Person SpecificationQualificationsWilling to undertake training identified by the manager as relevant to Clozapine service and patient needs Good standard of education to GCSE standard or equivalent Health and Social Care qualification ExperienceExperience of mental health needs and the impact on individuals and families Experience in working with vulnerable individuals. KnowledgeDemonstrate a sound knowledge of the Community Mental Health Framework and the NHS Long-term plan Confidence and ability to work with individuals in their homes Confidence and ability to work with individuals in their homes Ability to contribute to the planning, implementation, and review of collaborative care plans.
