Job summary We are looking for an enthusiastic, experienced Band 6 Learning Disabilities specialist nurse to join our busy Community Learning Disabilities Service in Harrow .This is a key role within the team and you will have both generic and specialist responsibilities. You must have a valid NMC registration with 4 years minimum post qualification experience in a mental health /learning disabilities setting. Together with other senior colleagues, you will be responsible for the day to day delivery of the service and representing the service as required. This post involves ensuring a high level of specialist provision across the community setting for adults with learning difficulties who have mental health issues and or challenging behaviour. Main duties of the job You will be required to provide supervision to junior staff and mentor students. Under the guidance of the senior nurse and team manager, you will be expected to lead and deliver comprehensive evidence based practice, as well as translating the Trusts' Research and Development Strategy into local service provision. You will be able to work calmly under pressure, be flexible and responsible to changing needs, able to deliver and support other staff in the safe delivery of services. The job-holder will be required to carry a caseload of service users within the Community Service. Where appropriate this includes roles of care co-ordinator, placement monitor, care manager and social supervisor. Although based in Harrow the post includes the requirement to manage cases outside the Harrow area. In addition to having a caseload, the post-holder will be required to adopt an effective and positive team approach and work with the other Care Co-ordinators to ensure effective team delivery of client/ carers care. It is expected that the post-holder will make a valuable contribution to multi-disciplinary team assessments, the formulation and implementation of MDT care plans, while working closely with individuals and teams in enabling recovery and independence.
