This role is ideal for a registered nurse with a passion for community care, clinical leadership, and staff development. You'll be instrumental in triaging care concerns, supporting care teams, and ensuring our clients receive timely, responsive, and high-quality care at home. Minimum 3 years post-registration experience in community or primary care Proven experience in clinical leadership, care coordination, or experience working with multidisciplinary teams and managing complex cases Strong clinical assessment and decision-making skills; ability to triage care concerns and implement early interventions Knowledge of safeguarding, risk management, and CQC standards Excellent communication, coaching, and interpersonal skills Competence in developing care plans and clinical protocols Compassionate and person-centred approach; proactive, solution-focused mindset Ability to lead, inspire and support care staff Commitment to continuous improvement and reflective practice DesirableLeadership or management qualification (e.g. ILM, Level 5 Diploma) Teaching or mentorship qualification Experience in service development or quality improvement initiatives Familiarity with NHS Continuing Healthcare or integrated care pathways Ability to analyse data and report on service outcomes Disclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
