Job Description

Overview The Royal Wolverhampton NHS Trust’s (RWT) Primary Care Network is made up of eight practices across Wolverhampton (all rated as Good by the CQC). We are part of a group of forward thinking practices working with RWT in our own network. We have a multidisciplinary team approach to working with ANPs; Practice Nurses; First Contact Physiotherapists; Social Prescribers and Pharmacists as well as a full admin team to support workflow, care navigation and call handling to enable more time to be released to provide effective patient facing care. Our surgeries use EMIS Web and Docman10.We are excited to be able offer a salaried GP vacancy to join our team under the Additional Roles Reimbursement Scheme (ARRS). This a great opportunity for a GP who is newly qualified. The postholder must be within 2 years of completing their Certificate of Completion of Training (CCT).We can offer 9 sessions per week and this will be a permanent contract.ResponsibilitiesTo provide a comprehensive range of clinical and triage duties at the agreed Surgeries within our Primary Care Network, which may also include working within our Enhanced Access Service.To undertake surgery consultations, home visits where necessary, and telephone queries from patients and health care professionals.Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.To undertake on-call duties as part of the Surgery on-call rota, where agreed. This may include requesting that patients present at the surgery, telephone consultations, home visits and triage.Work with patients to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects, and interactions.Assess, identify, and refer patients presenting with mental health needs in accordance with national and local guidelines.Assess the health care needs of patients with undifferentiated and undiagnosed problems.Screen patients for disease risk factors and early signs of illness.In consultation with patients and in line with current practice disease management protocols, developing care plans for health.Record clear and contemporaneous consultation notes to agreed standards including the recording of appropriate information for the Quality and Outcomes Framework Services.Collect data for audit purposes.Compile and issue computer-generated acute and repeat prescriptions.Timely assessment of incoming electronic and paper correspondence relating to both patient and non-patient information as required. Using the appropriate electronic systems.Complete insurance, and other reports relating to patients under our care.Undertake all the normal duties and responsibilities associated with a GP working within primary care and any other duties reasonably required by the practice.Support the development and implementation of new care pathways that align with ARRS objectives.Requirements / QualificationsNewly qualified GP (within the last two years), without a substantive post.PCN is open to new ideas and development of a model of Primary Care provision through Vertical Integration.Post is salaried with Royal Wolverhampton Trust; indemnity covered under the hospital scheme for GMS work; paid time for study and CPD.Context and Benefits The PCN is part of the Primary Care Directorate at RWT NHS Trust, enabling collaboration with managers and clinicians and aiming to make Primary Care more attractive as a specialty with improved working life for GPs.
