Being part of a specialist pharmacy team working within mental health services ensuring that the changing medicines management and pharmacy needs are being met in Eating Disorder Services, inpatient and community teams for CAMHS in Kent and Medway. ResponsibilitiesParticipation in multidisciplinary team meetings, consultant ward rounds or case conferences relevant to the specialty as appropriate to identify medication problems and provide advice. Reviewing medication charts/EPMA on wards and community teams to optimise patients9; treatment; ensure accuracy and appropriateness; monitor interactions and adverse effects; interpret medicines information to provide advice for individual service user circumstances. Giving prescribing advice to medical staff and non-medical prescribers. Advising on medicines risk management issues such as the reduction of medication incidents and ensuring compliance with medicines legislation. Providing medicines optimisation advice to medical, nursing and other staff and medicines information to all service users, carers, and staff. Liaising with GPs, PCNs and community pharmacists; involvement in discharge planning to ensure community medication needs of service users are fulfilled. Providing a medicines reconciliation service for patients with complex needs. Providing advice and information to service users and carers and actively being involved in self-medication schemes to promote patients9; independence. Supporting the running of the near-patient testing clozapine clinic. Supporting the provision of an equal standard of pharmacy service across the Trust by proactively developing and implementing protocols and guidelines; providing supporting documents for the Medicines Optimisation Group when requested. Involvement in a Trust-wide medicines information service and contribution to the development of a comprehensive medicines management system within the Trust. Providing clinical supervision and support for less-experienced pharmacists, students, and supervise technicians when undertaking ward and team clinical duties. Undertaking clinical and practice research and audit that will influence practice within the Trust (e.g. POMH -UK). Dispensing medicines using the Trust labeling machine when required. Assisting senior pharmacists in developing clinical and other services within the Trust. Participating in the review process of all drug-related incidents and formulary development and review in the specialties. Writing and participating in the development of shared care guidelines where required for the specialties; writing and participating in the development of PGDs and training staff on the use of PGDs where required. Participating in, contributing to and taking minutes for clinical pharmacy meetings. Identifying problems with pharmacy supplies and reporting to the designated member of pharmacy staff or liaising with the contractor to rectify. Representing pharmacy at ward/team level meetings.
