Senior Physiotherapist - Intermediate CareAn exciting new opportunity for a experienced Physiotherapist has arisen to work as part of a therapy team at Carlton Court intermediate inpatient care unit based in Lowestoft.Are you someone who is passionate about rehabilitation and reablement and looking for an opportunity to develop your skills further? This could be the role for you.The post holder will be expected to provide specialist clinical skills to manage a caseload of patients using evidence based practice in order to achieve their goals.Main duties of the jobThe post holder will provide a high standard of care as part of a multi-disciplinary team to patients within the intermediate care service and patients own homes. The post holder will be working as an autonomous practitioner applying a high level of clinical reasoning to ensure that individualised care is delivered safely.The post holder will be responsible for delivering an Intermediate Care service for patients with highly complex needs within a ward-based setting however there may be a requirement to work across a wider community setting. Involve patients in the planning of care wherever possible, and act in the best interests of the patients at all times. Actively manage the patients admission, involving clinical experts, when relevant, to advise in the diagnosis and treatment. Proactive discharge management, ensuring patients are consulted and proper management of situations where patients need assistance in decision making.The post holder will adopt a case management approach to ensure that rehabilitation reablement is actioned and integration of care takes place.To assess, plan, implement and evaluate care and treatment, ensuring the highest quality of patient centred, goal directed care at all times, and with appropriate involvement and support of relatives and carers.To interpret and analyse clinical and non-clinical facts to form an accurate diagnosis and prognosis in a wide range of conditions, to recommend the best course of intervention and to develop comprehensive management strategies for the patients presenting condition.To enhance and develop the skills and dexterity, co-ordination and palpatory senses for assessment and manual treatment of patients. To be safe and competent in the use of therapy equipment.To refine treatment techniques in line with the most recent evidence of clinical effectiveness by evaluating and implementing key developments and promoting discussion within service.To provide complex information to the patients, clinicians and other agencies to assist in decision-making, using persuasive, motivational, negotiating, training, empathic and re-assurance skills.To work with a defined caseload of patients with a broad range of health needs, using knowledge to carry out assessments, using standardised tools, setting objectives and implementing appropriate treatment, as part of the holistic care plan.To formulate an individual management/treatment plan for each patient using clinical reasoning skills and utilising a wide range of physical, sensory and cognitive treatment skills following analysis of information received from client, carers and referrer and any other professionals involved in the patients care as necessary.To ensure own and other staffs interventions are evaluated and treatment is modified in order to measure progress and ensure effectiveness of intervention.To provide spontaneous and planned advice, teaching and instruction to relatives, carers and other professionals to promote understanding of aims of therapy to ensure a consistent approach to patient care.To have working knowledge of equipment, minor adaptations and resources including activities of daily living equipment, therapy and rehabilitation aids, moving and handling equipment and a basic knowledge of wheelchair provision in order to compensate for unresolved functional deficits, in line with local resources, guidelines and legislation.Proactive participation in multidisciplinary team meetings.To complete clinical documentation and electronic data entry to a high professional standard and in accordance with Trust policies.Promote confidence in the patient by means of appropriate professional standards of appearance, behaviour and attitudes.Act on unplanned situations using appropriate clinical judgement.Safeguard the confidentiality of all information regarding the patient and their family.LeadershipTo support the staff through the provision of clinical supervision, audit, appraisal, provision of support training and to be visible to staff, patients and relatives.To act as an effective role model to other staff, being clear about expectations and ensuring that safe patient care is central to all activities.To act as mentor to newly qualified, new staff and other learners.Provide clinical supervision/support to students and more junior members of staff.Actively participate in audit and research.Maintain and develop own clinical and leadership skills to support practice.Adhere to Health Care Professionals Council (HCPC) and The Chartered Society of Physiotherapists (CSP) Professional Standards and Conduct, Organisational Policies and guidance.Demonstrate a sound understanding of Clinical Governance and Risk Management and apply to work situation.To develop and support evidence to meet the Care Quality Commission (CQC) fundamental standards.Clinical GovernanceContribute to clinical governance reports covering all aspects of activity.
