Service Care Solutions are seeking an experiencedAdvanced Clinical Practitioner (ACP)to join an innovative and establishedFrailty Team. You’ll work autonomously at an advanced clinical level, delivering person-centred care to frail and vulnerable patients in their homes and communities, supported by a multi-disciplinary team.Location:Scarborough and surrounding areasContract Type:Fixed Term – 24 MonthsSalary:Competitive (Per Session, based on experience)Hours:Full-time / Part-time consideredReports to:Director of Community Services Based in our Scarborough, you will provide leadership in delivering holistic care that helps reduce hospital admissions and facilitates early discharges for complex patients.Key Responsibilities:Independently assess, diagnose, treat, and manage patients with complex needsPrescribe, refer, admit and discharge within scope of practiceLead service development, quality improvement, and audit initiativesCollaborate across sectors and services to ensure joined-up patient careEngage with strategic frailty initiatives across the systemRequired:Registered healthcare professionalMSc in Advanced Clinical Practice (or equivalent portfolio)Independent Prescriber qualificationExperience managing frailty, complex care or urgent community servicesConfident leader and educator with strong decision-making skillsKnowledge of current frailty frameworks, national policy and integrated models of careThis role offers a chance to be part of a nationally recognised model of community frailty care and to work at the forefront of innovation in integrated health services.Why Should You Join?This position offer the chance to be part of a dedicated, values-led organisation committed to deliveringintegrated, person-centred healthcare.
