District Nurse (Fixed Term - June 2026) - Clayton HouseWe are looking for a Fixed Term District Nurse to join our friendly,inclusiveand forward thinking District Team, working across the West Lancashire area to provide care andsupportto patients in community settings.Here at Clayton House, we arecommittedto continuous professional development, therefore we offer all our colleagues to attend additional courses such as Palliative care or Frailty to further improve their skills and knowledge.The team is based out of Clayton House in Burscough.Please note that this role is Fixed Term until June 2026.A full UK Driving License is required due to the role being community based.Some of your responsibilities will be:To work as part of the 24-hour district nursing service, under the direction of the District Nursing Team Manager / Leader, providing high quality holistic care to patients.The District Nurse role carries delegated responsibility for the assessment, planning, implementation and evaluation of evidence-based nursing to patients in their own homes.To assume delegated responsibility for the supervision of the team in the absence of the District Nursing Team Leader, when required.To succeed within this role, you will have:A Degree/Diploma in Adult NursingFirst level registered nurseExperience working as part of a multi-disciplinary team / multiagency working (Desirable)You need to have an understanding of undertaking complex assessments and the planning and evaluation of care packageExcellent Communication, both written and verbalEvidence of effective interpersonal skillsClinical Risk assessment skillsAbility to problem solve and make clinical decisions Understanding of NMC standards and implications to practiceAbility to work independently in community settingHigh clinical standardsWe have built organisation-wide committees in many specialist fields.Youll have the option to join your clinical specialism, networking with professionals from across the country.We gettogetherregularly, auditing andsharingstandard methodology for innovation and service re-design. Youll be a part of the pool of subject matter experts bringing together clinical and operational leads. You will be a part of quality improvements, research and development activity.
