Working across South and South West London, you will play a vital role in shaping care packages that truly change lives.About the RoleNurse Case Manager As a Nurse Case Manager, you will lead and oversee a caseload of adult clients requiring complex care in the community. Youll design personalised care plans, support and clinically upskill care teams, and ensure safe, high-quality, and compassionate care delivery. You will collaborate closely with CHC teams, commissioners, MDT professionals and families to deliver holistic, outcome-focused care.Key ResponsibilitiesDevelop, implement and evaluate tailored care plansDeliver clinical oversight, supervision and competency training to care teamsSupport clients with complex needs including ventilation, tracheostomy care, PEG and NG feeds, spinal injuries and neurological conditionsMonitor, assess and review clients physical and emotional wellbeingEnsure safe medication management and clinical interventionsBuild strong, professional relationships with families, CHC nurses and commissionersContribute to service improvement and uphold high compliance standardsCriteria for SuccessRegistered Nurse (RGN) with active NMC PINClinical background in complex community care or acute complex careRecent experience in tracheostomy and ventilator careEmpathetic leadership style with experience supporting HCAs or support workersExcellent communication and clinical assessment skillsSuitable for Band 5 or Band 6 nurses ready to progress into a leadership roleFull UK driving licenseBenefits PackageSalary up to £45,000 per year (DOE)Hybrid working: 1 day WFH, 3 days in the community, 1 day at head office (Wimbledon)MondayFriday, 9am5pm33 days annual leavePrivate pension schemeCompany pool car or travel cardMileage at 45p per mileExcellent learning & development pathwaysCompany mobile phone and laptopIf you're ready to advance your leadership career as a Nurse Case Manager, wed love to hear from you.
