Assessment and Treatment Plans : Assess patients' physical conditions, develop and implement individualized treatment plans, and adjust them as necessary to ensure optimal recovery. Multidisciplinary Collaboration : Work closely with other healthcare professionals, including nurses and doctors, to ensureholistic carefor patients. Documentation : Maintain accurate records of patient progress, adapting treatment plans as needed and ensuring that all documentation is in line with professional and organizational standards. Post-Discharge Support : Provide advice and support to patients and their families, ensuring a smooth transition topost-discharge careand continued recovery.Essential Requirements HCPC RegisteredPhysiotherapist. Minimum of 2 yearsexperience in an acute medical setting. Experience in rehabilitationandpatient recovery . Ability to assess, plan, and implement effective treatment plans for a range of acute conditions. Car driver (preferable)for travel between different sites (if applicable).
