Clinical Lead Community Complex Care Salary 48,000 Reading & Oxfordshire Hybrid: 3 days community-based | 2 days from home UK Driving Licence & Own Car RequiredIs this the role you are looking for If so read on for more details, and make sure to apply today.Sentra Care are working in partnership with our client, a recognised leader in the Community Complex Care sector, to recruit an experienced Clinical Lead to support adults and children with complex healthcare needs in the community.This is a highly autonomous, clinically focused role offering a true hybrid working model, with the opportunity to make a meaningful impact on client outcomes while maintaining a healthy worklife balance.The Roleyou will be responsible for managing a community-based caseload across Reading and Oxford, ensuring the delivery of safe, high-quality, nurse-led care.The role will consist of:3 days per week in the community, completing client assessments, staff training, touchpoints, reviews, and clinical oversight2 days per week working from home for care planning, governance, and MDT coordinationOccasional travel to London (every few weeks) for team meetings and trainingYou will work closely with Care Coordinators, operational teams, and senior clinical leadership to ensure robust, client-centred care delivery.Key ResponsibilitiesLeadership & CollaborationReport into the Regional Lead Nurse and work alongside Practice Educators, and Care CoordinatorsBuild strong relationships with clients, families, community teams, and specialist cliniciansWork with central resourcing teams to ensure a skilled and consistent care workforceService Delivery, Quality & GovernanceCreate, review, and update person-centred care plans and risk assessmentsProvide visible, hands-on clinical support in the communityConduct client reviews at least every 8 weeksParticipate in the nurse on-call rotaEscalate clinical and safeguarding concerns appropriatelySupport complaint xjdpvnf resolution and clinical investigationsEnsure compliance with clinical policies and regulatory standardsEssential RequirementsNMC Registered NurseBackground in ICU/ITU/HDU or complex continuing healthcareRight to Work in UK - They cant offer Skilled worker visas at this stageMust be based in Either Reading, Oxford or surrounding areasPost-qualification experience in acute clinical careExperience mentoring, supervising, and developing staffStrong communication, analytical, and organisational skillsAbility to work autonomously and manage a community caseloadFull UK driving licence and access to your own vehicle
