This role is pivotal in delivering safe, effective, and patient-centred Clinical pharmacist services within primary care.Find out exactly what skills, experience, and qualifications you will need to succeed in this role before applying below.What youll be doing:Managing repeat prescriptions, acute prescription requests, medicines reconciliation and transfers of careProviding structured medication reviews and telephone clinics to support patients with queries about their medicinesSupporting safer prescribing systems, QOF and enhanced services, audits, PINCER, and the Prescribing Quality SchemeBuilding strong links with community pharmacies, PCN pharmacy teams, and care homes to improve medicines safetyWorking as part of a supportive team to ensure high standards of patient careWhat were looking for:Must have experience working in a GP surgeryCPPE training completedIndependent Prescriber qualification (essential)Sound clinical knowledge with a strong understanding of prescribing and medication reviewsA collaborative approach with the ability to work autonomously within a multi-disciplinary teamThe package:Competitive salary, dependent on experience xjdpvnf NHS pensionOn-site parkingSupportive working environment with regular team collaborationFull time (37.5 hours) with some flexibility for the right candidateThis is a fantastic opportunity to join a well-run, supportive surgery that values development and teamwork.
