This is an exciting opportunity to shape a dynamic new service that ensures care at home packages remain person-centred, proportionate, and aligned with individual outcomes. As part of the Care at Home Review Team you will be working across both urban and rural communities.As an Occupational Therapist, you will bring your expertise in functional assessment, and environmental adaptation to the review of care at home packages.
