This role involves working with people with mental ill health, substance misuse histories, and complex behavioural needs, delivering structured, person-centred support to promote stability, recovery, and risk reduction.About the roleIn this role, you will hold a caseload of individuals requiring intensive, coordinated support. You will develop detailed support and risk management plans, work closely with multidisciplinary teams, and ensure that safeguarding, compliance, and recovery-focused outcomes remain central to all interventions. The position requires resilience, strong professional boundaries, and a confident, trauma-informed approach.Key ResponsibilitiesProvide structured, person-centred support to individuals with forensic backgroundsDevelop, implement, and review support plans and risk assessmentsSupport individuals with substance misuse challenges, including drug and alcohol dependencyMonitor engagement and respond proactively to risks and early warning signsWork closely with MDTs, probation, healthcare, and social care professionalsContribute to safeguarding processes and report concerns appropriatelyMaintain accurate documentation and outcome reportingSupport individuals to build routines, life skills, and community connectionsAbout You<
