Newly Qualified Salaried GP - 4 sessions per week.Tower Hill Partnership is part of Pioneers Integrated Partnership Primary Care Network. PCNs are a key part of the NHS long-term plan. They provide the structure and funding for services to be developed locally in response to the needs of the patients they serve.Following the release of ARRS GP information and funding we are looking for a newly qualified (within 2 years and not having had a substantive post), enthusiastic, forward-thinking GP to join Pioneers Integrated Partnership (PIP) PCN to be based at Tower Hill Partnership.Main duties of the job We are a highly supportive PCN.To undertake duties governed by the PMS & GMS contract and Objectives of Direct and Local Enhanced services including:Face to face and telephone consultationsPatient Home visits and Nursing Home visitsChecking and signing repeat prescriptionsAdministration - dealing with queries, patient test results, referral letters NHS/private, task, emails and other paperwork and correspondence in a timely manner.Cover all the clinical management of the patients in surgery as appropriate including initiating investigations, reviewing results, making referrals to secondary care or to other providers as appropriate.About us Pioneers Integrated Partnership, West Birmingham.Number of Practices - x3Job responsibilities Clinical Responsibilities -In accordance with the Practice timetable, as agreed, the post-holder will be available to undertake a variety of duties including surgery consultations, telephone consultations, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork, reports and correspondence in a timely fashion.Triage patients presenting to the surgery by telephone, online and in person.Making professional, autonomous decisions in relation to presenting problems, whether self‑referred or referred from other health care workers within the organisation.Assessing the health care needs of patients with undifferentiated and undiagnosed problems.Screening patients for disease risk factors and early signs of illness.In consultation with patients and in line with current Practice disease management protocols, developing care plans for health and promoting healthy lifestyle strategies and education.Manage long‑term conditions and patients with complex problems.Recording clear and contemporaneous consultation notes to agreed standards.Collecting data for audit purposes and complying with QOF requirements.Delivering enhanced services.Check, manage & process patient test results.Refer patients to secondary care and/or relevant organisations and internally within the PCN (eg to additional roles members of the PCN team) as required.Compiling and issuing computer‑generated acute and repeat prescriptions (avoiding hand‑written prescriptions whenever possible) in line with local and national guidelines.Prescribing in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate.In general the post‑holder will be expected to undertake the duties and responsibilities associated with a GP working within primary care in a surgery on a PCN basis.Other Responsibilities within the Organisation -Awareness of and compliance with all relevant Practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety.A commitment to life‑long learning and audit to ensure evidence‑based best practice.Contributing to evaluation/audit and clinical standard setting within the organisation.Contributing to the summarising of patient records and read‑coding patient data.Attending training and events organised by the Practice or other agencies, where appropriate and suitable and relevant for your role.Keep up to date with current evidenced based approaches to patient care and service delivery, in line with NICE and national frameworks.Contribute to education and training of trainee members of staff and students.Attending and contributing to practice and PCN meetings.Person Specification QualificationsMBBS or equivalent medical degreeFull registration with GMCCertification of Completion of Training - JCPTGP/PMETBInclusion on Performers ListEvidence of a commitment to on‑going personal & professional developmentPersonal Skills and QualitiesAbility to communicate clear and effectively, both written and verballyExcellent organisational skillsAble to work calmly under pressureAbility to work on own initiativeIT skills including word processing and database workCommitted to quality and patient satisfactionCommitted to equal opportunitiesFlexibility & adaptabilityAbility to bring fresh ideasExperienceExperience of primary care through the GP Vocational Training Scheme.Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.#J-18808-Ljbffr
