[ within 2 years]The successful candidate will be primarily based at Creswell and Langwith Medical Centre. However, on occasion, you may be asked to work at one of our other DCHS General Practice sites in exceptional circumstancesPlease Note. INTERNATIONAL GRADUATES MUST BE QUALIFIED AND BE ON THE GP SPECIALIST LIST AS WELL AS THE GMC REGISTER.Main duties of the jobProvide face-to-face and telephone consultations for routine and urgent careProvide clinical care to patients as described in the job plan and in line with practice processes. This will include face-to-face and telephone triage.Action of clinical administration, including medical reports, test results, referral letters, and any other duties as required in a timely manner.Cover weekly on-call duties and support the service during absencesFortnightly rotational cover of the extended access service, which is a PCN DES requirement and takes place on Tuesdays 18:30-20:00Make clear consultation notes using System OneEnsure safe, evidence-based, and cost-effective care in line with Practice disease management protocols and NICE guidelines.Independently prescribe medicationsReview and manage patient medications following the appropriate policies and local clinical guidelinesWork with our clinical and non-clinical team to deliver holistic and patient-centred careActively participate in MDT meetings as requiredTake part in practice auditsActively attend and participate in educational and designated monthly training eventsTo lead on key areas within the practice, such as specific QOF domains, quality standards, and practice initiatives, as agreed with the clinical lead and /or practice managerProvide support and debrief for our trainee registrarsHave a flexible approach to meet the needs of the service.About usThe practice serves approximately 5500 patients across Creswell, Langwith, and surrounding areas.Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people's own homes and via virtual consultations.During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.Job responsibilitiesFor more information about this role, please review the job description and personal specificationPlease see attached Job Description and Person Specification.
