This role plays a key part in education, health and care assessment pathways.As a Health Practitioner / SEND Trusted Assessor, you will deliver trauma-informed and neurodevelopmental assessments, contribute to Education, Health and Care Needs Assessments, and provide therapy aligned with Education, Health and Care Plans. Apply Now for our Recruitment Team to review.DUTIESYour duties as the Health Practitioner / SEND Trusted Assessor include:Clinical Assessments: Completing assessments to inform Education, Health and Care Needs AssessmentsTherapy Delivery: Providing therapy as outlined within Education, Health and Care PlansReport Writing: Producing clear, transferable clinical reports and recommendationsAnnual Review Input: Contributing professional advice to Education, Health and Care Plan reviewsOutcome-Focused Intervention: Delivering goal-based, time-limited therapeutic workMulti-Agency Working: Collaborating with education, health and social care professionalsCare Transitions: Supporting discharge planning and transitions to appropriate servicesClinical Governance: Maintaining accurate records and complying with safeguarding and data protection standardsProfessional Development: Engaging in supervision, reflective practice and ongoing CPDCANDIDATE REQUIREMENTSESSENTIALRegistration with the HCPC in a relevant health professionPrevious experience in trauma-informed and neurodevelopmental practiceExperience completing assessments and contributing to SEND pathwaysProven ability to produce high-quality clinical reportsStrong communication, clinical judgement and IT skillsAbility to work independently and within multidisciplinary teamsDESIRABLEExperience with attachment-based and trauma-informed modelsA background in Education, Health and Care assessmentsExperience supporting children and young people with neurodivergenceBENEFITSFlexible part-time working with no evenings or weekendsHybrid working across home, office and community settingsAccess to ongoing CPD and specialist trainingSupportive clinical supervision and reflective team cultureThis role requires an Enhanced DBS checkPlease be advised that the company does not provide sponsorship for this position.
