Youll help them develop independence skills, understand their options, secure safe accommodation, and progress with education, employment, or training. You will manage a caseload, review Pathway Plans, and provide the practical and emotional support young people need as they transition into adult life.Key DetailsPay: 16.62 per hourHours: 37 hours per weekLocation: Wakefield, WF1 4XAContract: TemporaryWhat Youll Do Day to DaySupport young people aged 1625 who are preparing to leave care or who have left care.Manage and review each young persons Pathway Plan and Risk Assessment.Provide practical guidance with budgeting, benefits, utilities, managing money, and maintaining a tenancy.Help young people set up and maintain their accommodation, including providing intensive housing support where needed.Work closely with housing providers, social workers, health teams, voluntary organisations and other partners to coordinate support.Maintain accurate case notes and records using IT systems.Attend relevant meetings such as Looked After Reviews, CIN meetings and multi-agency discussions.Take part in the duty rota, responding to crises or urgent situations.Encourage young people to participate in service development and community activities.RequirementsEssentialNVQ-Level 3 (or equivalent).Experience working with or supporting vulnerable young people.Understanding of safeguarding and the challenges faced by care leavers.Knowledge of legislation relevant to leaving care.Strong communication skills, both written and verbal.Ability to manage a caseload and work independently.Good IT and record-keeping skills.Driving RequirementA full driving licence is required due to the mobile nature of the role.DesirableExperience working with looked-after children, local authority services, or voluntary organisations. xjdpvnf Understanding of child and young person development.
