We are supported by two experienced GP trainers andare actively involved in the supervision and development of GP registrars,foundation doctors, medical students, designated medical prescribers (includingnurses, paramedics, and pharmacists), and VTS nurses.We are also at the forefront of innovation within our Primary Care Network (PCN), leading the development of an advanced model for acute presentations through our PCN Acute Hub Pilot, supported by a wide multidisciplinary team.If you are committed to delivering high-quality patient careand have an interest in medical education or research, we would be pleased tohear from you.Job responsibilitiesJob responsibilitiesUndertake all the duties associated with therole of a General Practitioner required by the GMS contract, including patientconsultations, duty doctor cover and home visits. This should take in toaccount physical, psychological and social factors when diagnosing illness,treating diseases, tending to injuries and recommending treatment. Work inaccordance with the Practice rota system in place.Collect, record and maintain patientinformation, such as medical history, reports and examination formation is recorded on the practices clinical system and this informationshould be recorded accurately, using templates and read codes and in a timelymanner. Comply with all the objectives of the GMS Contract, QOF targets, ICBrequirements arrangementsWork in accordance with Practice clinicalprocedures and protocols. All work within the practice is governed by protocolsand procedures alongside other additional governance requirements from outsideorganisationsRequest, perform and/or interpret tests andrecords, reports and examination information to assist in diagnosis ofpatients condition. Prescribe or administer treatment, therapy, medication,vaccination and other appropriate medical care to treat or prevent illness,disease or injury. Monitor patients condition and progress and re-evaluationand/or change treatments as necessaryRefer patients to medical specialists or otherpractitioners appropriately, when necessary and in a timely manner ensuringcompliance with protocols and procedures for referralPromote health education in conjunction withother health care professionals and have an awareness of the local health andsocial plete all allocated and self-generatedclinical administration.Carry out clinical audits as pletion of reports for solicitors, insurancecompanies and other third parties as required.Liaise with other health care professionals whenrequired, including support to the nursing team during minor illness clinicsAct in a way that protects the confidentialityof patients and maintains the integrity of the patient/Practice relationship.This includes compliance with the Practices Confidentiality Policy andCaldicot guidance.Recognise the importance of effectivecommunication within the team and communicate effectively with team members,patients, their carers and other colleagues and health care professionals.Take an active role and contribute to Practicemeetings where appropriate.Support the compliance of the Practice Healthand Safety policy. Work safely at all times in accordance with legislative andpractice policies and procedures.Participate in the GP revalidation and appraisalscheme.
