Meon Health Practice - Salaried GP - Permanent (min 4 sessions)We are looking for an enthusiastic,dynamic GP to add to our brilliant and diverse primary care team of socialprescribers, pharmacists, physiotherapists, nurses and healthcare assistants,advanced nurse practitioners, physician associates, care coordinators, salariedGPs, retainer GPs and GP partners.Meon Health Practiceis an innovative and high achieving, but more importantly, friendly andwelcoming practice on the western edge of Fareham. We have three sites which sit at the centreof their respective communities, while immediately around us the South Downs (forland-based GPs) or the Solent (for water-based GPs) are outstandinglybeautiful.We offer a flexibleand modern general practice working day, which might include remote working orstaggered start and finish times. Youcould provide traditional primary care or spend time with our integrated teamsin urgent care, complex patients or those patients in residential care. There willbe opportunities to develop your clinical leadership skills within the practice.The salary will becompetitive and dependent upon your experience, and includes indemnity, sixweeks of annual leave and a week of study leave.Main duties of the jobThe post-holder will make themselves available to undertake a variety of duties, including surgery consultations, triaging telephone consultations, queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashionMaking professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisationAssessing the health care needs of patients with undifferentiated and undiagnosed problemsScreening patients for disease risk factors and early signs of illnessDeveloping care plans for health in consultation with patients and in line with current practice disease management protocolsProviding counselling and health educationAdmitting or discharging patients to and from the caseload and referring to other care providers as appropriateRecording clear and contemporaneous consultation notes to agreed standardsCompiling and issuing computer-generated acute and repeat prescriptionsPrescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriateIn general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.To hold a list of patientsAbout usMeon Health Practice is made up of 3 sites - The Highlands Practice, Jubilee and Whiteley Surgery.
