This will be to remotely monitor patients who are in a Care Home or Semi-Housebound to support their GP practice. Patients will initially have an assessment to create a care plan and ensure we have all the information needed for safe admission onto the service. Patients will have access to observation equipment and links to an app to input their readings which will alert a clinician if there are any problems or readings are out of range.Main duties of the jobWe are looking for forward thinking, motivated clinicians who have a passion for new ways of working whilst providing safe patient care. The clinician will provide support monitoring alerts and may require visiting patients to provide support in their home environment, if necessary, this maybe due to tech issues or setting up equipment and training when first onboarded.
