Social Care Assessor required to work in a community social work team based within the Royal Victoria Hospital in Belfast. This role will involve working with the social work team to implement care plans, maintain individual caseloads, and provide advice and support to elderly patients and their families regarding available resources. You will also be responsible for preparing reports and maintaining accurate records, as well as liaising with other healthcare professionals in relation to referrals and ongoing treatment plans for patients. The hours of work are Monday Friday, 9am-5pm, with an hourly rate of 14.06. This post will initially last for 3 months, with the possibility of extension.
