In this role, you will play a vital part in coordinating patient appointments and ensuring the seamless delivery of home health services. Your contributions will directly impact our mission of providing high-quality care that supports individuals in aging with dignity.ResponsibilitiesThe Scheduler ensures schedules are prepared to meet demands for service and visit assignments in accordance with the plan of care and state and federal regulations. As a Scheduler you will:Support direct care of patients by providing field staff scheduling and adjusting schedules to accommodate physician ordersReview scheduling request report and make changes to the schedule as requestedProcess therapy reassessment warning and ensure functional assessments are scheduledManually enters visits completed by therapy staff and ensures functional assessments are flagged appropriatelyPrepare, run, and distribute reports from clinical software systems to healthcare team as neededBe responsible for ensuring the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly CareRequirementsProven experience in medical administrative support or scheduling roles.Two years' administrative experience in an office environment, preferably in healthcareProficiency in Microsoft OfficeIf you are passionate about making a difference in home health care and have the skills to coordinate our services effectively, we invite you to apply today and join our dedicated team at Alpine Care Home Health!
