This role is essential to ensuring resident-centered, compliant, and holistic care planning in the post-acute and long-term care setting.The ideal candidate is an experienced RN with strong assessment skills, exceptional documentation abilities, and a sharp eye for detail. This is an administrative nursing role that blends clinical expertise, leadership, and education.✅ Key Responsibilities:Coordinate the interdisciplinary resident assessment processEnsure accurate and timely completion of MDS assessments in accordance with company, state, and federal guidelinesLead the development and ongoing updates of Comprehensive Care PlansMonitor residents to ensure care plans are followed and updated as neededIdentify and report care plan variances to the DON and Care Plan TeamServe as a clinical and regulatory resource for the interdisciplinary team?
