What You'll Do Develop individual care plans with input from the individual and their family, based on the individual's personal preferences and choices. Assist individuals and families in coordinating, selecting, and overseeing their own support. Work collaboratively as part of a multidisciplinary team to ensure timely integration of services. Develop and implement behavioral guidelines and interventions; track and analyze related data. Participate in training that promotes sensitivity to cultural and socioeconomic diversity. Meet face-to-face with assigned individuals as medically necessary per their Individual Plan of Service. Provide consultation to direct care staff regarding clinical and program-related matters, as applicable. Provide individual and group therapy sessions, as assigned; assist in developing and facilitating group sessions. Work Schedule Standard Hours: Monday-Friday, 8:00am-5:00pm, on-site Flexible scheduling available based on caseload and program needs On-Call Rotation: Participation required; Rotates one week on, one week off On-Call Pay Enhancements Weekdays (Mon-Fri): $50 per on-call shift Weekends (Sat-Sun): $100 per on-call shift Holidays: $150 per on-call shift Qualifications Minimum Education: Bachelor's degree in Social Work, Psychology, or a related Human Services field required; Master's degree in Social Work or Psychology preferred. One or more of the following credentials strongly preferred: Licensed Bachelor Social Worker (LBSW) or Limited Licensed Bachelor Social Worker (LLBSW) Licensed Master Social Worker (LMSW) or Limited Licensed Master Social Worker (LLMSW) Licensed Psychologist (LP), Limited Licensed Psychologist (LLP), or Temporary Limited Licensed Psychologist (TLLP) Social Service Technician (SST) Certification Preferred Experience: 1-2 years of experience working with individuals with developmental disabilities (DD) or mental illness (MI). Prior knowledge of the community mental health system and/or Medicaid-funded services. Experience working with electronic medical records (EMR).
