Purpose and Impact: The position is responsible for the execution of the SAMHSA SBIRT grant objectives and serves as the primary point of contact for community agencies, stakeholders, and SAMHSA. This position will provide daily oversight of the grant and is responsible for overseeing the implementation of the project activities, internal and external coordination, development of annual reports and submission of all project deliverables to SAMHSA. Essential Functions: Monitors achievement of grant-stated goals and objectives, including monitoring program budget and reporting data quarterly to SAMHSA and project This is accomplished through monthly data collection and analysis, regular status update meetings, and ongoing communication. Works collectively with state and community departments and systems to improve collaboration and care coordination in the geographic catchment Creates solutions where barriers have been identified. Establishes new partnerships and works to strengthen existing relationships. Develops and updates policies, procedures, and protocols specific to the project and develops any required agreements to document follow-up and care Coordinates information sharing and helps resolve barriers to program Serves as the primary reporting and programmatic liaison to Ensure oversight and execution of SOR grant objectives and compliance with mandatory project deadlines as outlined in contract and pertinent state rules, regulations and statute. Ensure in partnership with network service providers that the State Opioid Response (SOR) Grant System Priorities as outlined in the guidance from the Department are achieved to include: Establishing Emergency Department Bridges (Hospital Bridge) to community-based methadone or buprenorphine (Medication Assisted Treatment) prescribers throughout the northeast region of Florida. Establishing County Jail Bridges to community-based methadone or buprenorphine (Medication Assisted Treatment) prescribers throughout the northeast region of Florida Ensure access to Buprenorphine in all counties in the LSFHS Catchment area. Analyze and plan for sustainability, Monitor and improve retention in care. Coordinate, communicate, and promote data sharing among community stakeholders and agencies according to approved protocols. Monitor progress in meeting performance measures. Trend data to identify gaps in care and potential opportunities for innovative programming. Using data to drive decisions, work with network service providers to identify and develop innovative programming to meet the community need Engage in the development of the agency’s plans and programs as a strategic partner. Evaluate and advise on the impact of long-range planning, introduction of new programs/strategies and regulatory action. Serve as the primary point of contact for administration of the Paramedicine Program, Jail Bridge, and Hospital Bridge. Interpret and administer pertinent federal and state rules, regulations, and statutes in accordance with sound clinical principles. Compile detailed reports regarding utilization management data, trends and quality improvement needs. Education / Experience: Bachelor's Degree Advance degree preferred in human services or related field with at least 2 years of experience implementing and coordinating grant projects in multiple counties. Ability to solve problems and manage projects to meet goals and objectives according to established Skills: Proficient in Microsoft Office Excellent research, critical thinking, writing, editing, data analysis, presentation, and communication skills. Attention to detail and ability to organize and present information in an easily understandable format with a strong understanding of its impact. Ability to meet critical deadlines on concurrently occurring projects. Aptitude for prioritizing responsibilities and managing time effectively. Willingness to work both independently and collaboratively. Professionalism and diplomacy via all modes of communication, whether in person, by telephone or in writing.
