We are seeking a knowledgeable and detail-oriented MDS Coordinator (RN) to join our team and ensure compliance with federal and state regulations while enhancing resident care outcomes.Job SummaryThe MDS Coordinator is responsible for the overall coordination and management of the resident assessment process, including timely and accurate completion and submission of the Minimum Data Set (MDS), Resident Assessment Protocols (RAPs), and Care Plans in accordance with HCFA and State of Connecticut requirements.Key ResponsibilitiesAdministrative FunctionsCoordinate, complete, and transmit MDS assessments in accordance with state and federal regulationsMaintain MDS/care planning policies and proceduresDevelop and monitor a quality assurance program for assessments and care plansParticipate in discharge planning processesCare Plan & Assessment ManagementChair the Interdisciplinary Care Plan TeamEnsure accurate development of individualized care plans based on MDS assessmentsConduct initial and periodic resident assessmentsMonitor significant changes and ensure appropriate follow-up MDS is completedInterdisciplinary & Committee ParticipationCollaborate with all departments and participate in key facility committeesAssist in maintaining communication and compliance between care team membersFacilitate resident and family participation in care planningPersonnel & Staff DevelopmentProvide education and training on MDS and care planning processesAssist with ongoing in-service programs and training needsFoster positive working relationships across departments and care teamsResident Rights & ComplianceUphold resident confidentiality and rights, including advance directivesRespond to and document resident complaints or concerns related to assessment or care plansEnsure assessments align with resident goals and preferencesQualificationsEducation & LicensureNursing Degree from an accredited program requiredActive, unencumbered RN license in the State of ConnecticutExperienceMinimum 2 years of supervisory nursing experience in a hospital or long-term care facilityPrior experience in MDS coordination highly preferredSkills & CompetenciesThorough knowledge of MDS 3.0, HCFA regulations, and CMS guidelinesExcellent leadership, communication, and organizational skillsAbility to work independently and collaborativelyStrong assessment and documentation skillsFamiliarity with CEI software and electronic submission systemsBenefitsSign-On BonusCompetitive salaryHealth, dental, vision, and life insurancePaid time off (vacation, sick leave, holidays)401(k) retirement plan with employer match
