We are seeking an individual to seek out eligible individuals and referral sources in need of the various LGI services - Low vision, Health Center, Adult Day Health Care, Behavioral Health Services, and Vocational Rehabilitation. Qualifications: Highly motivated individual with a Baccalaureate or master's degree in public health, marketing, or related fields. Two to three years of experience preferred. The individual must understand health services, community service systems and be able to communicate health information. Excellent verbal, written and interpersonal skills required. Must be comfortable conducting presentations to individuals and groups of all ages. Must be proficient in word, excel, PowerPoint and web searching. Position Summary:  Seeks out eligible individuals and referral sources in need of the various LGI services - Low vision, Health Center, Adult Day Health Care, Behavioral Health Services, and Vocational Rehabilitation. Works with Director of Outreach and Support Programs to create schedules for outreach presentations and visits in the community. Collaborates with other departments within LGI for outreach presentations and materials. Visits and maintains contacts with community centers, hospitals, behavioral health agencies, ophthalmology offices, health homes, subsidized housing, human service agencies, etc. to inform the population of various LGI services available and encourages referrals. Works closely with referral sources to ensure that referrals meet eligibility in terms of vision status and qualifying criteria, and that evaluation visits are set up. Maintains a database of contacts, including referring doctors, clinics, health homes, social service and other community representatives, referrals received, and all follow-up performed. Maintains confidentiality of registrant protected health information. Responsible for following the LGI Workforce security policy; will access information systems and ePHI authorized by Clinical Authorization only. Gains knowledge about and keeps up to date regarding the full range of Lighthouse Guild programs and services as well as the needs of each department represented by Outreach. Represents the agency in conferences and advocacy groups as assigned. Monitors the tele-support program registration process and conduct follow-ups with new registrants. Maintains the tele-support program database, including participant information, session recordings, release forms, and guest speaker contacts. Schedules guest speakers for tele-support programs. Conducts research and gathers relevant information for the Family Support e-newsletter. Performs other duties as deemed necessary and appropriate by the Director of Outreach and Support Programs. ADA REQUIREMENTS: Ability to sit at a desk and use a computer. Ability to talk on the telephone and cell phone. Ability to move from place to place within the office. Ability to transport oneself to and within on-site offices and other off-site facilities.
