At Mosaic, we relentlessly pursue opportunities that empower people to live a meaningful life in a caring community.   As a Mosaic at Home Coordinator, you will play a vital role in ensuring individuals receive personalized support and have a voice in their own lives.    What You'll Do: Provide Direct Support: You will provide direct care and support, averaging 15-20 hours per week. This includes assisting individuals with daily living, facilitating community integration, and attending occasional evening and weekend events (approximately one per month). Case Management & Monitoring: Manage a caseload of 22 individuals in Independent Living and Intermittent Services. You will complete monthly home visits and oversee the services delivered by their Direct Care Workers (DCWs) to ensure compliance with contracts and individual support plans. Coordinate Care: Schedule and attend client medical appointments, ensuring all health needs are addressed. Develop and Maintain Documentation: Write and regularly update individual support plans, programs, and assessments. You will also monitor DCW documentation to ensure accuracy and compliance with services provided. Participate in On-Call Rotation: Join a rotating on-call schedule with other team members. You will be on-call for one week at a time, approximately once every six weeks, to provide support for urgent situations.   SCHEDULE: Days, 40hr/week, with a semi-flexible schedule, hybrid position, occasional evening and weekend hours SALARY RANGE: $57,500 - $60,000 CLOSING DATE: September 24th, 2025 by 5pm   Commitment to Mosaic Values: At Mosaic, we believe in creating a workplace where everyone has the chance to contribute and succeed. What You'll Bring:  A Bachelor’s degree is preferred;  A minimum of two years of related work experience is required.  Previous experience working with people with intellectual disabilities is a plus.  Valid Driver's license. Passion for Mosaic’s mission.  Strong communication and relationship-building skills.  Experience in behavior management and knowledge of regulations.  Excellent planning and problem-solving skills.  Computer proficiency.
