Schedule: Part-time Shift : Days Location: 100 HEALTH PARK DR, Louisville, 80027 The role you'll contribute: The Sepsis Coordinator is a health care professional who works in collaboration with the medical and hospital staff to provide multidisciplinary care to the septic patient. The Sepsis Coordinator provides clinical, professional expertise for patients, families, staff members and to healthcare providers. The scope of practice includes ensuring organized care is provided to the patient with sepsis with a focus on health promotion and disease prevention. The Coordinator also functions as an educator and coordinates in critical care/sepsis patient care management and outcomes. The value you'll bring to the team: Evaluates the delivery of care systems and assures compliance with quality measures through concurrent and retrospective review, identification of deficiencies, and implementing corrective actions through staff education. Assists in providing staff education related to sepsis and serves as a resource to the Education Team. Develops and maintains research protocols and serves as co-chair of the monthly sepsis meeting, drafting the agenda and minutes. Coaches and mentors staff, communicates information to team members, and participates in ongoing staff development.
