Position Overview The Hospice Inpatient Clinical Coordinator (HICC) plays a critical role in supporting hospice admissions and inpatient care delivery. This position is responsible for developing and strengthening relationships with hospital partners, evaluating patients referred for hospice enrollment, and providing General Inpatient (GIP) level hospice care. The HICC conducts comprehensive clinical assessments, facilitates hospice admissions, collaborates closely with interdisciplinary teams, and ensures compliance with all applicable state and federal regulations. This role combines strong clinical expertise with relationship management and care coordination in a fast-paced inpatient environment.
