Position Summary:The Manager of the LTCH Patient Clinics is responsible for the overall coordination, supervision, and clinical management of all LTCH Patient Clinics as well as the Sterilization arena at Hebrew SeniorLife. This leadership role ensures the delivery of safe, efficient, and high-quality care while maintaining compliance with all hospital licensure, infection control, and regulatory standards.Reporting to the Chief Nursing Officer and Vice President of Patient Care Services, the Manager of the LTCH Patient Clinics oversees clinic operations across multiple disciplines and specialties, providing direct supervision to clinic staff, including Clinic Coordinators, Dental Assistants, and any other pertinent staff working within the LTCH Patient Clinics. This role includes scheduling, staffing oversight, performance management, education, and ongoing competency evaluation.The Manager of the LTCH Patient Clinics is the main clinical liaison between MD specialty consultants (e.g., cardiology, dental, podiatry, neurology, ophthalmology, and others) and internal care team members including RNs, NPs, MDs, and PCAs. The Manager of the LTCH Patient Clinics ensures that the orchestration of all required clinical services is efficient, timely, and fully aligned with best practices and licensure requirements.In the Sterilization Arena, the Manager of the LTCH Patient Clinics ensures strict adherence to sterilization protocols and infection control guidelines. This includes oversight of spore testing, equipment maintenance, compliance documentation, and continuous staff education to ensure the safe reprocessing of instruments used in podiatry, dental, and other specialty care clinics.Using the full scope of nursing practice—including assessment, planning, implementation, and evaluation—the Manager of the LTCH Patient Clinics provides hands-on patient care when needed and supports clinic staff in delivering patient-centered, evidence-based services. The Manager of the LTCH Patient Clinics also supports interdisciplinary collaboration, proactive care coordination, and quality improvement efforts across all clinic services.A key contributor to the patient experience and operational success of the LTCH Patient Clinics, this leader serves as a clinical mentor and role model, fostering professional growth, accountability, and a culture of excellence in line with the mission and values of Hebrew Rehabilitation Center.II. Position Responsibilities:Patient Care Support:Ensures all care delivered within the Patient Clinics aligns with the scope of professional nursing practice, established safety standards, and core clinical competencies. The Manager of the LTCH Patient Clinics plays a critical role in direct patient care support, team education, and interdisciplinary collaboration.Clinical Assessment and Triage• Assess and triage patients receiving care from multiple consulting physicians; manage urgent clinical issues promptly and appropriately during clinic visits.• Navigate and coordinate clinical needs prior to patient visits to optimize time spent with MD consultants (e.g., obtaining ECHOs, EKGs, labs).Support for Specialty Care Services• Provide clinical support for patients seen across a wide range of specialties, including but not limited to: dental, endocrinology, cardiology, neurology, podiatry, ophthalmology, optometry, dermatology, orthopedics, physiatry, and wound care.• Apply in-depth nursing knowledge across these disciplines to effectively support patient needs and consultant requirements.Procedure Assistance and Direct Care• Assist with clinical procedures such as lumbar punctures, eye exams, pupil dilation, intraocular pressure checks, and other nursing-related clinical support tasks.• Identify, acquire, and utilize necessary resources to ensure the delivery of safe, high-quality, and efficient care.Patient Care and Clinical Support• Monitor patient flow to ensure timely care delivery in accordance with hospital licensure requirements and clinical best practices.• Prepare exam rooms by setting up treatment tables, instruments, equipment, and supplies; dispose of sharps and other materials per infection control protocols.• Obtain and document patient vital signs (blood pressure, temperature, weight) in advance of provider examinations.• Enter and track medication orders; administer medications to long-term chronic care patients, as appropriate and within scope of practice.• Collaborate with patients and families to support appropriate care planning and treatment decisions.• Secure and store all medications in compliance with safety and regulatory standards.Interdisciplinary Collaboration and Communication• Serve as the primary liaison between MD consultants and internal clinical teams, including RNs, MDs, NPs, Nursing Leadership, and allied health professionals, to ensure clear and effective bidirectional communication and care continuity.• Collaborate with the broader care team—including physicians, nurse practitioners, social workers, and clinic coordinators—to meet patient needs and monitor clinical outcomes.Education, Evidence-Based Practice, and Quality of Care• Educate Clinic Assistants on quality care principles, patient safety, and customer service to maintain high standards of patient experience.• Adhere to current best practice guidelines and evidence-based interventions to promote optimal clinical outcomes.• Demonstrate initiative in identifying patient care needs and proactively seek out resources or interventions to address them.Oversight of LTCH Patient Clinics:Clinic Operations and Team Supervision• Oversee daily operations of all LTCH Patient Clinics, providing direct supervision to Clinic Coordinator and Dental Assistant to ensure smooth workflow and high-quality care.• Lead and support clinic teams to ensure effective, efficient, and patient-centered care is delivered across all clinic settings.• Schedule patient appointments for physician clinics at Roslindale and Dedham locations, as needed, to maintain continuity of care.• Collaborate in the development and implementation of improved work processes and systems to support clinic efficiency and performance.• Maintain compliance with hospital policies, procedures, regulatory requirements, and HIPAA standards; assist in developing or updating policies as needed.Patient Relations and Issue Resolution• Proactively address and resolve patient and family concerns in a timely, respectful manner.• Support and guide clinic staff in managing patient-related concerns, escalating complex issues as necessary.Quality and Reporting Responsibilities• Participate in quality improvement efforts by collecting and monitoring clinical and operational data to ensure high standards of care.• Initiate and complete required reports, including:o Department of Public Health (DPH) reportso Incident reports and concern resolutionso Infection control reports and line lists•​ Support and report on quality and process improvement initiatives through the QAPI (Quality Assurance and Performance Improvement) process.Supervision of Sterilization Arena:• Ensure Compliance with Sterilization Protocols: Monitor and enforce adherence to sterilization protocols and procedures across all applicable areas to maintain safety and regulatory standards.• Training and Education: Conduct regular reviews and provide education to direct reports on end-to-end sterilization processes, including the transportation, processing, and proper removal of instruments from sterilization equipment.• Performance Oversight and Feedback: Deliver consistent feedback and oversight to staff involved in sterilization and dental waterline maintenance, ensuring full compliance with hospital licensure requirements within all LTCH Patient Clinics.• Instrument Sterilization and Testing: Oversee the sterilization of podiatry and dental instruments, including the execution of autoclave spore tests. Ensure the accurate and timely maintenance of sterilization logs.• Competency Management: Ensure direct reports complete required core competencies related to sterilization and waterline maintenance in a timely manner.• Infection Control Collaboration: Serve as the primary liaison with Infection Control to implement and monitor compliance with recommended protocols and improvements.• DPH Rounds Leadership: Act as the lead representative during Department of Public Health (DPH) rounds and actively follow up to ensure that all Infection Control (IC), safety, and compliance standards are being met in all LTCH Patient Clinics.Financial and Administrative Oversight:Budget and Financial Management• Assist in the development, allocation, and monitoring of Patient Clinic budgets across both Roslindale and Dedham campuses.• Review and verify clinic invoices to confirm billed appointments; provide supporting documentation, make necessary billing adjustments, and track billing activity.• Generate and analyze monthly reports (e.g., appointment outcomes, invoiced services, medication orders) to support Continuous Quality Improvement (CQI) initiatives.• Collaborate with the Administrative Director to assess and optimize consultant utilization based on evolving patient needs; advocate for new specialists when appropriate.Equipment and Facilities Oversight• Track maintenance schedules and service needs for clinical equipment and instruments to ensure optimal functionality and safety.• Coordinate with medical equipment vendors and representatives to support the safe and timely use of medical devices (e.g., pacemaker machines).Staff Supervision and Development• Conduct annual staff performance evaluations to support development, accountability, and performance improvement for LTCH Patient Clinic Staff and HSL employed Dental Staff.• Oversee all LTCH Patient Clinic staff, HSL employed Dental Staff and any other staff who perform sterilization at both campuses.• Deliver educational programming, lead staff meetings, and facilitate quality improvement initiatives to ensure staff engagement and clinical excellence.• Promote ongoing professional development by modeling professional behavior and critical thinking, and by taking accountability for one’s own learning.Administrative Support and Patient Coordination• Coordinate patient transportation services in partnership with the Volunteer Services department to ensure timely and safe access to care.• Complete assigned special projects and other duties as required to support overall clinic operations and patient care delivery.IV. Qualifications:• RN license required, BSN preferred.• Previous long-term care, acute or skilled nursing experience required.• 3-5 years of supervisory experience required.• Strong interpersonal skills required to work effectively with elderly patients and be responsive to their needs.• Requires the ability to work closely and effectively with physicians and other staff.• Demonstrate initiative in problem solving and independent work.• Must have superior customer service skills.• Must have critical assessment skills to manage urgent care patients.• Should have the ability to demonstrate sensitivity to patient comfort and privacy, e.g., prompt response to patient requests• Must be professional, proactive, collaborative, conscientious and results-oriented individual.
