Minimum Requirements Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.Active Kentucky physical therapy license required to start the assignment.BLS certification required from AHA or ARC.Experience providing physical therapy in a Home Health environment preferred.Current, valid driver’s license and reliable transportation to travel to patient homes.Ability to perform patient evaluations, develop treatment plans, and document according to home health agency standards.Electronic medical record proficiency preferred. Previous experience with home health documentation systems is preferred.
