Minimum Requirements Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.Active Illinois Physical Therapy license required to start the assignment.BLS certification from AHA or ARC may be required.Experience working in a skilled nursing facility or long-term care setting preferred.Experience providing evaluation and plan of care development for geriatric patients preferred.Ability to manage caseloads typical of a skilled nursing facility and to document in electronic medical records preferred. Assignment Details Facility Type: Skilled Nursing Facility.Shift Name: Days.Shift Details: Days 08:30–16:30.Schedule: 5 shifts per week, 8 hours per shift.Assignment Length: 13 weeks.Guaranteed Hours: 35 hours per week.
